Form 2112 - Required to make changes to students file
All changes for both cash and subsidy clients must be in writing, all changes once submitted will be reviewed and complete with a decision in 24 to 48 hours after submitting the request for change. All request for change will require a 2 week - 3 week notification, depending on the change / changes being made. For un enrollment a 3 week notice is required for all other changes a 2 week notice is required.
